APC Curriculum (Course) Change Form
Used to make changes to/propose new course, new course description, course number change, course prerequisites, drop course, etc.

APC Program Change Form
Used to request changes to majors, minors, & learning communities.

Application for Writing (W) Course Designation
Course change form should accompany this.  Submit to Director of Writing first.

Application for Intercultural (I/D) Course Designation
Course change form should accompany this.

Application for Technological Competency (TC) Course Designation
Course change form should accompany this.

Please note that forms must be complete in order for them to be considered by APC.  Department faculty must sign the forms and vote tallies must be indicated.
