All students must complete the following and turn in to the faculty/staff person coordinating the form collection for the EYH course they are participating in:

1. Photocopy of the photo page of passport
2. Signed Travel Waiver Please note that a parent signature is required on this form and there is a box that needs to initialed on the second page *
3. Signed Parent Waiver and Release for Off-Campus Programs ALL students must have this form signed by a parent, even if they are over 18
4. Signed Behavior Code for Off-Campus Programs
5. Off Campus Medical Form (Students must include a copy of their health insurance card with this form)
6. Sexual Harrassment Policy Form
